Permission to use your member feedback.
This member information sheet confirms the terms on which you as a member (you) agree to allow us (Network), as the Discounts for Teachers and Healthservice Discounts membership schemes operator, to use information or materials, featuring or referring to you, including any image(s) of you, which you have provided to us and/or approved (as prepared by us) (Content) for the specified purpose described below.
1. In consideration of Network sending you the agreed survey participation benefit (£50 ode card credit), you confirm that:
1.1 You grant Network permission to publish the Content on our website(s) and/or on our own or third-party social media sites or platforms targeted at UK readers/audiences (Specified Activities) for the purpose of promoting the Discounts for Teachers or Healthservice Discounts membership schemes to both existing and prospective members (Specified Purpose).
1.2 You do not object to Network:
(a) storing copies of the Content for as long as necessary to fulfil the Specified Purposes (including storing your contact details on our database in case we need to contact you); or
(b) sharing the Content on our own or third-party social media sites or platforms targeted at UK readers/audiences. The privacy policies of any such third-parties, acting as data controllers, will apply.
2. In consideration of Network sending you the agreed further participation benefit (£250 ode card credit), you confirm that:
2.1 You grant Network permission to publish the Content on our website(s) and/or share all or any Content with third party website operators (such as online newspapers) and/or on our own or third party social media sites or platforms targeted at UK readers/audiences (Specified Activities) for the purpose of promoting the Discounts for Teachers or Healthservice Discounts membership schemes to both existing and prospective members (Specified Purpose).
2.2 You do not object to Network:
(a) storing copies of the Content for as long as necessary to fulfil the Specified Purposes (including storing your contact details on our database in case we need to contact you); or
(b) sharing the Content with any third-party website operators [approved by you] (such as online newspapers) and/or on our own or third-party social media sites or platforms targeted at UK readers/audiences. The privacy policies of any such third-parties, acting as data controllers, will apply.
3. You have the right to withdraw the consent you have given at any time by writing to Network at [email protected] or Unit 3, Glasshouse Business Park, Glasshouse Road, Wigan, Lancashire, England, WN3 6GL. This will not affect the lawfulness of any processing carried out by Network before you withdraw your consent.
4. Network and any third-party data controller (such as online newspapers) with whom we share your data with your consent is the controller and responsible for the personal data comprised in the Content or contact details.
5. Network will:
5.1 Ensure that it complies with the requirements of all legislation and regulatory requirements in force from time to time relating to the use of the Content and your contact details; and
5.2 Process the Content in accordance with the privacy notice below.
6. These terms and any dispute or claim (including non-contractual disputes or claims) arising out of them shall be governed by and construed in accordance with the law of England and Wales.

We, Network Digital Marketing Limited (Network), are the controller and responsible for your personal data.

- Content case study - content on you as an active member of the Discounts for Teachers or Health Service Discounts scheme, which may include: your name, job title (e.g., teacher, nurse), place of work (e.g., a named school or hospital), town or county in which you work and any comments on the scheme benefits or your member experience which you have agreed we may use for scheme marketing and promotional purposes; and
- Contact Details solely to enable us to communicate with you, being your name, phone number(s) and email address(es).
7.2 Purposes for which we will use your personal data
We have set out below, in a table format, a description of all the ways we plan to use your personal data, and which of the legal bases we rely on to do so. We have also identified what our legitimate interests are where appropriate. References to Specified Activities, Specified Purpose, Content and Contact Details are as defined above.
